About Depression and Anxiety
Checklists to Identify Depression and Anxiety

A variety of simple checklists are often used to help people and/ or their doctors to identify depression and anxiety. It is often helpful to complete one of these tests and share the results with your doctor or another mental health professional. The Depression checklist, K10 and SPHERE questionnaire (12 items) are all scales of psychological distress. Please note, we keep no record of your test results or identity.

Depression checklist
This is the checklist used by doctors to determine whether a person has major depression.

K10
This is a commonly used brief test for psychological distress.

SPHERE questionnaire (12 items)
This checklist has been used in Australia to assist general practitioners identify psychological or physical symptoms commonly experienced by people with depression or anxiety.
